Christopher Leesman, DO (he/him) - Medical Director
Dr. Leesman, board certified in family medicine, co-founded Transformative in 2022 with Tracy. In addition to his operational administrative duties, he currently serves as Medical Director and clinician providing ketamine-assisted psychotherapy and esketamine (Spravato®) treatments. Tracy Lenee Bluhm, LMFT (she/her) - Therapy Director
Tracy has a background in midwifery and primary care and co-founded Transformative with Dr. Leesman. She has supported individuals and couples with holistic, systems based therapy as well as ketamine-assisted psychotherapy. In addition to her clinical work, Tracy serves as Therapy Director, Clinical Supervisor, and operational administration. Read Tracy’s bio here.

Melanie Cookson (she/her) - Practice Administrator
Melanie is a human resource and practice manager experienced in risk management and fluent in healthcare operations. Her professional career spans over 25 years supporting organizational vision and driving consensus and collaboration across the functional and human aspects of the healthcare continuum. Her experience as practice manager of a DBT practice during COVID catalyzed an enthusiasm for integrated behavioral health.
Melanie’s professional focus is alleviating the administrative burden for practices and providers to nurture their individual and collective well-being and support the health of the community through each client. Her dedication to fostering inclusive workplaces informs all of her encounters. Professional interests include global healthcare transformation through population health.
Outside of work, Melanie is deeply committed to community service, actively contributing to Habitat for Humanity, Big Brothers Big Sisters, and environmental conservation efforts through the Southern Oregon Land Conservancy and Sierra Club.
In her free time, Melanie enjoys reformer pilates, yoga, meditation, reading, travel, museums and art galleries, independent films and time spent in nature including hiking, fishing and sailing.
She holds a Bachelor of Arts degree in Political Science and Art History from the State University of New York at Stony Brook.

Felicia has seven years of experience in office and medical administrative roles. Her passion is serving and assisting others, reading, and spending time with family and friends.

Pilar (she/her) - Intake Coordinator
Pilar is an experienced intake coordinator with a strong commitment to supporting individuals as they take their first steps toward mental health care. With many years in administrative and client-facing roles, she excels at creating a welcoming, organized, and compassionate entry point for clients seeking services.
Pilar is dedicated to ensuring that each person feels heard, supported, and guided through the intake process with clarity and care. She brings professionalism, empathy, and a steady presence to her work—helping connect clients with the right resources and contributing to a smooth, high-quality experience for both clients and clinicians.

Paige, CCMA (she/her) - Medical Assistant
Paige has been a Certified Clinical Medical Assistant for over 10 years in both Family Medicine and Mental Health settings and has enjoyed getting to know and working with patients through their health care journey. Raised in the Willamette Valley, she enjoys spending time with her family and dogs, reading, traveling, learning to crochet and soaking up the sun whenever possible.
