Psychiatry Medical Provider
Transformative Health and Wellness is an integrative outpatient health clinic that combines a variety of modalities to support whole-person health. We offer an array of mental health treatments as well as ketamine assisted psychotherapy and are looking to expand our services for the community. We are looking for a creative and motivated full or part time medical provider (Psychiatrist MD/DO, PMHNP, PA-C) to support this vision. Our vision statement is focused on approachable, grounded, humanistic, sustainable whole-person care for the community, multiple options of care for diverse clients, supporting the creativity of each provider while maintaining a team approach. Qualifications:
Graduation from an accredited school of medicine with a PMHNP, MD or DO degree and an approved residency program in psychiatry
Current Oregon license
Board certification by national board
DEA certification
Responsibilities:
Performing assessments, consultation and follow-up appointments
Collaborate with an integrative multidisciplinary team
Maintain a manageable workload that supports sustainable work/life balance and conscientious care for clients

Mental Health Therapist
We are hiring pre-licensed and licensed therapists, part and full-time, seeing clients in person and telehealth. We have flexible positions that can fit your employment needs. We provide comprehensive, compassionate care for the community and are dedicated to supporting growth for all our practitioners. We offer a combination of modalities, specializing in ketamine assisted psychotherapy, individual, couples and family therapy, massage and more. As a learning facility, we offer supervision with various teaching styles to create an in-depth and comfortable learning environment. Preferred Experience:
A minimum of one year working in direct: evidence-based modalities, DSM-V diagnosis, crisis assessment and intervention, and caseload management.
Ability to work effectively both independently and as a member of the integrated team.
Ability to balance work and self-care to maintain a positive mental attitude.
Ability to provide evidence-based practices and other approaches that promote health and well-being.
Ability to effectively evaluate client progress and adjust treatment interventions as needed.
Ability to perform assessments and diagnosis, and write and maintain treatment plans, documentation, progress records, and discharge criteria in a timely manner.
Excellent communication skills with clients, colleagues, and administration.
Cultural sensitivity and ability to work with people from diverse cultural backgrounds.
Abil ity to maintain confidentiality and comply with all HIPAA regulations.
Ability to engage in respectful and collaborative problem solving.
Professional appearance appropriate to the position.
Specific Duties:
Provide a wide range of clinical services, including individual therapy, family therapy, group therapy, ketamine assisted psychotherapy, case management, and consultation to referred clients.
Conduct diagnostic assessments and determine and develop treatment plans based on identified needs.
Maintain records in accordance with clinic standards.
Meet with supervisor for clinical and administrative supervision as directed.
Attend scheduled meetings.
Participate in training and professional development activities to meet credentialing and program expectations.
